【Function and Indications】
Mainly used for malignant lymphoma and cancerous pleural, pericardial and abdominal effusion.
【Usage and Dosage】
Because this product has obvious local irritation and is easy to cause tissue necrosis, it is only for arterial, venous and intracavitary administration.
Intravenous push
5-10mg each time, 1-2 times a week, total amount 30-60mg, treatment interval is 2-4 weeks, each dose is dissolved with 10ml of normal saline, and slowly pushed through the latex tube that is infusing 5% glucose solution. The injection should continue for a certain period of time to reduce the stimulation to the vein.
Arterial injection
5-10mg each time, once a day or every other day, dissolved with normal saline. Intracavitary injection: 10-20 mg each time, dissolved in 20-40 ml of saline, injected into the chest or abdominal cavity after the fluid is drawn. The body position should be changed several times within 5 minutes after injection to make the drug solution evenly distributed in the cavity. Once every 5-7 days, 4-5 times for a course of treatment.
Dosage for blocking the lower body of the abdominal aorta
0.2 mg/kg each time, 2-3 times a week, a total of 60 mg for a course of treatment. Method: After blocking the abdominal aorta with a belly band, gauze ball and sphygmomanometer balloon, the drug is quickly injected from the upper limb vein, and the belly band is removed after 10-15 minutes.
[Precautions]
1. Do not let this drug leak out of the blood vessel during injection; once it leaks out of the blood vessel, 0.25% sodium thiosulfate or saline should be injected subcutaneously locally and cold compress should be applied for 6-12 hours.
2. During the medication period, white blood cells and platelets should be checked 1-2 times a week.
3. Nitrogen mustard is extremely unstable after dissolution. It needs to be freshly prepared when used. Dissolve it in 10ml of normal saline and inject it intravenously immediately.
4. Alkylating agents have mutagenic or teratogenic effects. Pregnant women should use them with caution.
5. It is carcinogenic. Long-term use of nitrogen mustard increases the risk of secondary tumors.
6. This product can increase uric acid in blood and urine and reduce plasma cholinesterase, which interferes with diagnosis.
7. This product should be freshly prepared and used within 10 minutes. It cannot be used for subcutaneous injection, intramuscular injection or oral administration.
[Adverse Reactions]
Local reactions
Nitrogen mustard has a strong irritating effect on local tissues. Repeated injection of veins can cause phlebitis and thrombophlebitis. Leakage of the drug outside the blood vessels can cause local swelling, pain, and even tissue necrosis and ulcers.
Gastrointestinal reactions
Loss of appetite, nausea, vomiting or diarrhea, among which vomiting is more prominent. Ondansetron or metoclopramide and dexamethasone can be used to stop vomiting.
Myelosuppression
It is the dose-limiting toxicity of nitrogen mustard, which can cause a significant decrease in white blood cells and platelets. The lowest value appears 7 to 15 days after medication and can be recovered in 2 to 3 weeks.
Others
It may cause dizziness, fatigue, hair loss, amenorrhea, infertility, etc.
