【Function and indications】1. This product is used for non-small cell lung cancer, breast cancer, ovarian cancer, lymphoma, etc. There are many data on the treatment of NSCLC with this drug. The efficacy of single drug application is 14% to 33%. The efficiency of combined application with cisplatin is 36% to 52%.
2. This product also has a good effect on breast cancer, with an efficacy between 35% and 52%. The efficacy is further improved when combined with doxorubicin.
【Model and specification】Vinorelbine for injection: 10mg per vial; 50mg
【Usage and dosage】The dosage method is generally 25~30mg/㎡, intravenous drip, once a week, and 2 consecutive times as a course of treatment.
【Precautions】Bone marrow suppression is more obvious, mainly leukopenia, which usually recovers within 7 days. Thrombocytopenia and anemia are less than 2%. Neurotoxicity is mainly manifested as decreased tendon reflexes (about 25%) and constipation (17%~41%). Some patients may have intestinal paralysis, mostly ovarian patients. 2%~6% of patients have numbness of fingers (toes), but the incidence is much lower than that of VCR and VDS. Nausea, vomiting and hair loss are also rare (<10%). This drug is irritating to veins and should be avoided from leaking out of blood vessels. After the injection, 100~250ml of normal saline should be given to flush the veins.
[Contraindications] 1. Those who are allergic to vinorelbine. 2. Those with a white blood cell count below 1000/m3.
[Pregnant women use] Pregnancy and lactation.
[Drug interactions] The main effect is to bind to tubulin, thereby hindering the formation of microtubules in cells during mitosis. NVB is a cycle-specific drug with an effect similar to VCR. When the concentration is >12nmol, it can block the G2–M phase. In addition to the microtubules of mitosis, it also has affinity for axonal microtubules, so it can cause neurotoxicity, but it is lighter than VCR.
After entering the blood, most (80%) are bound to proteins, and the number drops to 50% after 96 hours. Drugs labeled with radionuclides are shown to be excreted in urine at a rate of less than 12% within 72 hours, and in humans and apes, 50% to 70% are excreted in feces (3 to 4 weeks). Therefore, patients with abnormal renal function can use this drug, but because it is mainly excreted from the bile duct, people with bile duct obstruction should reduce the dosage.
