【Functions and indications of injectable actinomycin D】
1. It has outstanding efficacy in treating Hodgkin’s disease (HD) and neuroblastoma, especially in controlling fever;
2. For the initial treatment of non-metastatic choriocarcinoma, the cure rate of injectable actinomycin D alone is 90% to 100%, which is similar to the effect of MTX alone;
3. It is also effective for testicular cancer, and is generally used in combination with other drugs;
4. Combining with radiotherapy to treat children’s Wilms tumor (Wilms tumor) can improve the survival rate, and it is also effective for Ewing sarcoma and rhabdomyosarcoma.
【Usage and dosage】
Intravenous injection: Generally, adults take 300 to 400 μg (6 to 8 μg/Kg) per day, dissolved in 20 to 40 ml of 0.9% sodium chloride injection, once a day, 10 days as a course of treatment, two weeks of interval, and a total of 4 to 6 mg for one course of treatment. Injectable actinomycin D can also be injected intracavitary. In combined chemotherapy, the dosage and time are not yet unified.
【Precautions】
When injectable actinomycin D leaks out of the blood vessels, it should be blocked locally with 1% procaine, or 50-100mg hydrocortisone should be injected locally, and cold wet compress should be applied;
Injectable actinomycin D should be used with caution in patients with low bone marrow function, history of gout, liver damage, infection, history of urate kidney stones, recent radiotherapy or anticancer drugs.
【Adverse Reactions】
1. Bone marrow suppression is a dose-limiting toxicity, with thrombocytopenia and granulocyte reduction, the lowest value is seen 10-21 days after administration, especially platelet decrease.
2. Gastrointestinal reactions are more common when the dose exceeds 500μg each time, manifested as nausea, vomiting, diarrhea, and a few oral ulcers, which begin several hours after medication and are sometimes severe, and are acute dose-limiting toxicity;
3. Hair loss begins 7-10 days after administration and is reversible.
4. A few cases of gastritis, enteritis or skin erythema, desquamation, pigmentation, liver and kidney function damage, etc., can be reversible.
5. Leaking blood vessels cause significant damage to soft tissues.
[Contraindications]
People with bleeding tendency should use injectable actinomycin D with caution or not, and those with a history of chickenpox should not use it. Injectable actinomycin D has mutagenic, teratogenic and immunosuppressive effects, and is contraindicated for pregnant women.

Pregnant women and lactating women should use injectable actinomycin D with caution.
