【Function and indications】
Tamofen is used to treat advanced breast cancer and ovarian cancer. In clinical treatment of breast cancer, the effective rate is generally 30%. The effect is better in estrogen receptor-positive patients (49%) and worse in estrogen receptor-negative patients (7%). It can be used by both premenopausal and postmenopausal patients, and the effect is better in postmenopausal patients and those over 60 years old than in premenopausal and young patients. From the perspective of lesion site, the effect is better in skin, lymph nodes and soft tissue, and worse in bone and visceral metastasis.

【Usage and dosage】
Regular adult dose Oral administration 10~20mg each time, once in the morning and evening each day.
【Clinical research】
It is a partial agonist of estrogen and has estrogen-like effects, but the intensity is only 1/2 of estradiol. Animal experiments have shown that it can promote vaginal epithelial keratinization and increase uterine weight, and can prevent fertilized eggs from implanting and delay ovulation. It competes with estradiol for estrogen receptors. This drug receptor complex can translocate into the cell nucleus, preventing the activation of chromosomal genes, thereby inhibiting the growth of tumor cells. It is rapidly absorbed after oral administration, and the blood concentration reaches a peak 4 to 7 hours after oral administration. The elimination half-life is 7 days. After 7 days of continuous administration, the blood level stabilizes at a high level. Its metabolites are N-desmethyl tamoxifen and 4-hydroxy tamoxifen. Most of it is excreted in the feces in the form of conjugates, and a small amount is excreted in the urine.
[Precautions]
1. Use with caution in patients with visual impairment, liver and kidney dysfunction before taking the drug.
2. For patients who take this product for a long time and are at risk of thromboembolism, blood counts should be checked regularly during treatment. If there is bone metastasis, blood calcium should be checked regularly at the beginning of treatment. During long-term treatment with this product for more than 2 years, liver function should be tested regularly.
3. When abnormal vaginal bleeding occurs, you should see a doctor immediately and undergo a comprehensive examination. The drug should be discontinued in case of heavy vaginal bleeding.
4. Patients treated with this product have an increased risk of endometrial cancer, so a careful gynecological examination should be performed.
5. The use of this product may cause sudden functional ovarian cysts, menorrhagia and irregular uterine bleeding. Therefore, if this drug must be used before menopause, antigonadotropin drugs should be taken at the same time.
[Adverse reactions and contraindications]
Gastrointestinal reactions, loss of appetite, nausea, vomiting, diarrhea. Continuous anti-estrogen effects, facial flushing, menstrual disorders, amenorrhea, vaginal bleeding, etc. Neuropsychiatric symptoms, headache, dizziness, depression, etc. Visual impairment. 【Drug Interactions】
When used in combination with warfarin or any other coumarin anticoagulant, the anticoagulant effect may be significantly increased, so patients should be closely monitored when used in combination. When used in combination with cytotoxic drugs, the risk of thromboembolism increases. In the early stage of treatment with this product for patients with bone metastases, if drugs that can reduce renal calcium excretion, such as thiazide diuretics, are used at the same time, the risk of hypercalcemia may be increased.
